Conveying a Concern


Please use this online form to convey any concerns you have about any member of the University of Richmond community, to include but not limited to students, faculty, staff, and visitors. Faculty - complete a report below for each student who appears to be at risk of earning a final grade of D or F or is struggling academically in other ways and/or who is exhibiting behaviors that raise concern. All concerns will be routed to the appropriate office(s) who are in the best position to respond. Your concerns do NOT necessarily have to be connected to a particular incident/event.

All emergencies that occur on the campus of the University of Richmond should be reported to the University Police in person or by phone by calling 9-1-1, (804) 289-8911, or (804) 289-8715. It should be noted that when using a cell phone to call University Police, you should dial (804) 289-8911 as dialing 9-1-1 directly may route them to another police agency. If you should be routed to another police department, you need to advise them of your specific location at the University of Richmond so that they may dispatch that information to UR Police.

If you are reporting a concern related to sexual misconduct (including sexual harassment, nonconsensual sexual intercourse, nonconsensual contact, dating/relationship violence, sexual exploitation, stalking, or hazing), please complete the Sexual Misconduct Incident Reporting Form. Another option is to share your concern directly with the Deputy Title IX Coordinator, Tracy Cassalia, at (804) 289-8464.

For 'Student of Concern' reports ONLY:  If this student meets with a CAPS counselor now or in the future, please understand that the CAPS counselor may be informed about the concerns expressed in this form, and may inform/discuss these concerns with the student.  

All employees are required to report all known information regarding Title IX incidents and concerns.
